A company is looking for a Care Manager (RN) Diabetes.

Key Responsibilities

Evaluate member needs and recommend care plans to address barriers to care
Develop and coordinate ongoing care plans, collaborating with providers and community resources
Monitor member progress and provide education on disease processes and healthcare options

Required Qualifications

Degree from an Accredited School of Nursing or Bachelor's degree in Nursing
2 - 4 years of related experience
Registered Nurse (RN) State Licensure and/or Compact State Licensure required
