A company is looking for a Bilingual (Spanish) LPN Personal Care
Coordinator I.
Key Responsibilities
Provide monthly care coordination and advocate for patient health
needs
Identify patient-specific problems, goals, and interventions based
on clinical assessments
Maintain patient chart compliance through accurate documentation
and updates
Required Qualifications
Fluent in Spanish and English
Licensed and currently registered as a Licensed Practical Nurse in
a COMPACT state
LPN degree from an approved program required
Pass background check
